FDU Soccer Camps are for all boys & girls, ages 5 - 17
SUMMER 2010 CAMP APPLICATION,
BROCHURE AND WAIVER FORMS:
There are two applications that are currently available.
The first application is a 1-page application. The second application is a 2-page brochure that would normally be printed on the front and back of a single sheet of paper. The brochure has an abbreviated application form included. We strongly suggest using the first 1-page application. It is more complete.
Please remember that if you are going to mail the application directly to us, that payment is due with the application.
Faxed applications can not be processed until payment is received.
Finally, there is a “waiver” form that must be completed. This "Waiver" may be completed prior to camp, or it may be completed on the first day of camp. Campers MAY NOT participate in the camp until a waiver has been signed and received by the FDU Soccer Camp Staff.
All documents are in Adobe Acrobat format or Microsoft Word format.
If you choose to print out the Adobe forms, you must have Acrobat Reader installed on your computer.
(Note: Most computers made since 2000 already have Acrobat Reader installed)
